Over the next few months, SAB will share weekly emails full of information, resources, reminders, tips and important details that you may find helpful throughout the enrollment and preparation process. Please refer to a log of these emails below. You can reference a schedule of upcoming communications here to see what you can expect to learn over the coming weeks.  (Note that this schedule is subject to change) You may also reference Enrollment resources and key contact info below. Summer Course Residence Hall Agreement — (for students living in the Residence Hall)

Medical Consent Form

Physician’s Medical Report/Immunization Record — This document must be downloaded and completed by the student’s medical provider. If possible, please upload a copy of the completed document to the Medical Documents upload form in your Powerschool Forms area. This can also be submitted to the contact information provided on the form.

Student Permissions Worksheet (Residence Hall students only)

Digital ID Photo Guidelines — You are required to upload an image in the Powerschool Forms area which will be used for student identification cards/photo facebooks for our faculty.  This document outlines the guidelines for creating that photo.

Parents may access all required enrollment forms in the SAB Powerschool Portal.  Visit the link below to login.
