Summer Course Enrollment Agreement
Summer Course Residence Hall Agreement — (for students living in the Residence Hall)
Physician’s Medical Report/Immunization Record — This document must be downloaded and completed by the student’s medical provider. If possible, please upload a copy of the completed document to the Medical Documents upload form in your Powerschool Forms area. This can also be submitted to the contact information provided on the form.
Student Permissions Worksheet (Residence Hall students only)
Digital ID Photo Guidelines — You are required to upload an image in the Powerschool Forms area which will be used for student identification cards/photo facebooks for our faculty. This document outlines the guidelines for creating that photo.
